Thanks for all your helps. After many testing and found out that Confirmation sent will be triggered by the Requestor upon clicking on the Inbox file in TCode: SBWP.
It applied to all other confirmation sent.
There is another Step on Rejection where there is no "confirmation sent" in the flow.
